Minimum Qualifica1ons: • Baccalaureate Degree or equivalent work experience. • At least one year of experience in a case coordina1on environment. • Desire to work with people with disabili1es. • Ability to travel up to 20% or more as needed within Colorado Springs and surrounding coun1es to meet with individuals and groups to present informa1on and training. • Proficient with computers, including Word, Excel, PowerPoint, Publisher, Outlook, Teams, and Zoom. • Demonstrates The Independence Center’s core values of Inclusion, Adaptability, Person First, Collabora1on, and Integrity as well as its essen1al team virtues of Humble, Hungry, and Smart.

Preferred Qualifica1ons: • Lived experience in the military • Knowledge of Home & Community Based Services and Veteran preference. • Experience working with the VA.

JOB CODE: 1010300100 Rev Mar 2021 Veteran Coach, VIC (Cont.) • Assist Veterans with obtaining access to services and supports to help them live independently in the community, increase their self-advocacy skills, and improve their quality of life. • Monitor services and problem-solve for a caseload of Veterans. Conduct func1onal assessments on ac1vi1es of daily living for long-term services and supports; review available documenta1on; and apply criteria and regula1ons specific to requests for services. • Facilitate development, implementa1on, and documenta1on of Veteran-directed support and spending plans. Monitor billing to ensure Veterans are within budget. • Work in partnership with Financial Management Services (FMS) to assist Veterans in comple1ng employer and employee paperwork and submi`ng 1mesheets. • Implement non-compliance policy and risk mitigation plans as needed. • Collaborate with military en11es, community partners, and external vendors to best serve Veterans. Promote The IC through posi1ve representa1on and communica1on. Aaend weekly case review mee1ngs and required staff meetings. • Communicate and collaborate with other team members on process improvement, system development, and strategic planning. • Work with Outreach in outlying areas to educate staff and facilitate mee1ngs with poten1al consumers. • Maintain timely, complete, and accurate documentation in database. • Responsible for professional development and posi1on-related training. • Par1cipate in The IC’s ongoing quality improvement ac1vi1es.

Physical/Environmental Requirements: • Occasionally lid, push, pull, up to 50 lbs. • Home health field environment with a high level of contact with others, oden requiring personal protec1ve equipment. • Office environment requiring ongoing computer use and extended amount of 1me si`ng or standing. • Internet access to work remotely if protocols are issued for remote work. • Must maintain an acceptable Motor Vehicle Record (MVR) as determined by The IC’s liability insurance carrier.
